We may collect information you provide through the case review form, including your name, contact details, accident category, location, timing, injury and treatment status, and any summary you choose to provide. We may also collect technical information such as IP address, browser information, referring page and campaign parameters.
TrustedForm consent certification
Our case review form uses ActiveProspect TrustedForm Certify. When the form page loads, TrustedForm may record the page content, technical metadata and interactions with the form to generate an independently verifiable consent certificate. The resulting certificate URL is submitted with your request and may be shared with participating service providers for compliance, quality assurance, fraud prevention and audit purposes. TrustedForm may process information according to ActiveProspect’s own privacy practices.
How we use information
We use submitted information to process your request, assess whether participating service providers may be able to assist, communicate with you, prevent fraud and abuse, maintain site security, comply with legal obligations and improve our services.
How information may be shared
We may share information with participating attorneys, legal service providers, intake partners, technology vendors and other service providers involved in responding to your request. You may ask us to update or delete certain personal information, subject to legal, security and recordkeeping requirements. You may opt out of marketing texts by replying STOP and may request that we stop marketing calls or emails.
Security and retention
We use reasonable administrative, technical and organizational safeguards designed to protect information. No online system can guarantee absolute security. We retain information for as long as reasonably necessary for the purposes described here, legal compliance, dispute resolution and fraud prevention.
Children’s privacy
This service is intended for adults and is not directed to children under 13. We do not knowingly collect personal information from children under 13.
